Health & Medication Forms

Here are the required health and medication forms for families to complete and submit as needed. Both forms must be completed by a physician prior to any medication being administered at school. The North Carolina Health Assessment Form is also required for new students entering TMSA for kindergarten or transferring from an out-of-state school. Completed forms may be submitted to the front office or to our medical staff.
